Assessment Corrections Form

Description

This Assessment Corrections Form is used frequently in my classroom as a way to enhance student understanding by requiring them to learn from their mistakes. When students earn a score below 80%, I allow them to use this form to make corrections. The form is very clear and self-explanatory, and requires students to identify their mistakes and then re-solve, providing evidence, work, and/or explanations. My students often "Ah ha!" moments when completing this form and avoid making the same mistake twice.

Besides allowing students to learn from their mistakes, I also use this form to help kids bring their grades up. For each problem that a student completes correctly, they earn half credit back towards their final score. The ability to earn points back is very motivating for students.

Students get one week to complete the form so that it's done while knowledge is still fresh and before other assessments occur. This form also requires a parent signature on the back.
