Assignment Checklists that include Subject (Reading, Math, Science, Social Studies), a place for Assignment Name, Date, Student Names and boxes to check off when they have turned things in to be graded. There is also a space at the bottom to list what each column is for. I often used these for packets, and I would write the grade in the box for each section of their packet.
