I apologize for not having a preview. I had technical difficulties.
This form makes it easy for students with fine motor and handwriting issues to show work clearly. There is space for 12 problems per sheet (if you run them front and back). I like to copy this form on white paper so that students who need it do not feel different from their peers.
I have a companion form for all students to use when correcting assignments and tests. It is identical to this one except for the title, "Assignment/Test Correction Form." I like to run it on brightly colored paper so that corrections are easy to find. Students like the bright paper, as well. Please see my other products for this free download.
Both of these forms were adapted from one that I found from another teacherspayteachers seller. I find that this is most useful for math, but can be adapted for any subject.
