I wanted a template to open communication between my kiddos school therapist and myself. It was helpful to me when I was working as a school SLP to have not only the students goals, but their progress towards those goals. To have information about motivators for students was even better!! I would have loved to have a letter like this from my students' previous therapist! I hope you find this helpful in opening that conversation for you!
