This form is for use during an Instrumental Recruiting session. We allow all our incoming band students to try mouthpieces for flute, clarinet, trumpet, trombone/baritone, some rhythm exercises for percussion and violin and cello. This helps us to best match student and instrument. We use this form to help us keep track of results.

I had to modify this a bit, but it saved me reinventing the wheel. I used it for six classrooms in two buildings so it really was time saving. I was able to move more quickly through testing and then enter the information into my computer later.
