This form allows teachers and students to peacefully brainstorm a solution to a continued behavior or work ethic problem and to communicate this problem to parents.

My students and parents appreciate that what is going home is in the student's words, not mine, and I like that the form forces the student to reflect on his/her behavior and develop their own plan for success.
