This is a fantastic tool to use as a part of your behavior management system. It includes questions such as "Why is this a problem?" and sentence starters such as "The choice I made is a problem because..." to support students in reflecting on their behavior. There is a Google Form and a Google Doc included in your purchase. I personally liked printing out the Google Doc version but Google Forms can also be a great way to keep the reflection sheets organized and easily distributed.
In my classroom, I use this as a tool prior to a call home. I use it as an opportunity to work with the student to support them and have them talk through it with me during break or lunch. It's not only a great way to support students but to also document how you are supporting the student if admin is requiring documentation prior to escalation for behaviors. I typically give a warning first, then use this reflection sheet, then I'll call home if behavior continues or is bad enough to warrant a call home. This reflection sheet is appropriate for most grade levels. I have personally used it for grades 6-8 and found it to be a very helpful tool.
