Description
This chart is used to help teacher and student monitor behavior daily. There are 5 pages, one for each possible behavior grade: E (excellent), V (very good), S (satisfactory), N (needs improvement), U (unacceptable). Each page explains, in student friendly language, what a student must do to earn that grade and what reward or consequence applies. All of this information can be easily edited.
The behavior chart needs to be assembled. I glue each page to a matching piece of construction paper and then glue all 5 pages together vertically and then laminate. Each student has a clothespin with their name, they start the day on S and can move up or down according to their behavior. At the end of the day, I write this grade in their agenda book.

Love the expectations being spelled out for each level. The only thing I wish was different was that it was is something other than publisher.
I can save it in another format like a PDF if you'd like and then email it to you that way. Or, if you need the name or some of the expectations changed I can make the changes on my end and email you the updated version. :)
