This reward chart can be used for behaviors, for increasing independent use of academic strategies, or for completion of activities. It is designed to be use both with guidance and independently. There is a spot for a reward menu, and a space to specify strategies to be used when focusing on the specific behaviors.

I have used this for social skills, for behavior management, and for reading strategies all with success. It is geared towards older elementary students, but could easily be modified for middle school students as well for independent monitoring.

The preview is in pdf format, but I am selling the Word document so that you will be able to modify it to your needs (add in the behaviors/skills and the strategies as well as reward menus).
