I am using this with a student who is an AVID reader and also has ADHD. I'm hoping that this will be as successful for you as it has been for me thus far. Each day, my student carries this on a clipboard from class to class with the expectation that they follow the guidelines in the top box. I am enjoying the reflection piece because this allows the student to have a conversation with me at the end of the day, if time, and/or their family at home. I have uploaded the .docx file so that you can tweak and edit as needed. Let me know of any changes that you've made - maybe that will make this contract work better.
