Description

This sheet enables teachers to track one student's behaviors for up to five dates. I used to track data on post-it notes, and as you can imagine it was a haphazard method of tracking data. So I created this form to keep my data organized. I added a place to jot notes so that I can put a few words with my tally marks. I designated one of my clipboards for behavior tracking, and it has not only helped me keep data organized, but it is a visual reminder to actually track the data.

  • Class Discipline Record: This sheet enables you to track the behaviors of many students in a class. I use this record for each class section I teach in middle school, but it can easily be used at all levels.
  • Student Discipline Record: This sheet enables you to track the behaviors of one student, and helps you set up a schedule of short conferences to keep the student on track. I only use this sheet with those few students who need a lot of support.

This behavior tracker will make it simple to track what problems certain students are having in class and pinpoint what there possible triggers are. At the end of each week, I will scan each paper and email the results to the parents of each child.
