This file is intended for behavior documentation, and it goes with the parent communication behavior chart that is listed on my TpT site also.

Each time a child "pulls their clip" or "changes their color," they mark it down on their behavior chart AND on this form. There is a code for each behavior infraction, and they record the code that corresponds with the infraction onto the chart.

This form stays in a binder that travels with the class throughout the day. Each child gets their own page for the 9 week period. On the chart, there is a column for each class, so you can tell if a child is having a problem in a certain class or if it's everywhere.

This file is editable, but you'll need to download the font "Pineapple Delight" from dafont.com for the formatting to look the same.
