●►The Behavior Questionnaire will:
▪Help staff to focus on goals and concerns regarding a specific student
▪Identify what triggers the unacceptable student behavior
▪Identify what motivates a particular student to misbehave
▪Improve meetings regarding student with a more focused discussion and clarity for parents/guardians specific staff concerns
●►The Accommodation Checklist will:
▪Help focus on strategies already attempted and possible new strategies.
▪Document what has worked in the past
▪Document what has failed in the past
●►The Student Questionnaire will:
▪Involve the student in the process
▪Increase your chances of success
▪Identify what student sees as problem, and motivational factors he/she need to change behavior
▪Ensure understanding among all participants of the plan
●►Student Reinforcement Survey will:
▪Help determine what will work as positive reinforcement in the behavior plan that is developed
▪Generate ideas to aid in development of intervention plan
●►Additional Uses:
▪Part of your school/program pre-referral process
▪Staff complete for assistance request from Intervention Specialist, Special Education Teacher, School Social Worker, School Psychologist or Administration
▪Administration utilize for frequent behavior referrals
▪Excellent data collection tool
