Description

One thing more teachers need to do is to keep records of behavior interventions that they have tried for students. Whenever we have IEP meetings or when we are evaluating a student for special education services, teachers often bring up student behavior and say that they have tried multiple things and we need to put the student on a behavior plan as well. However, they often do not have documented attempts and data to back that up. By using this form, teachers can get a collective idea as a team of what things have worked and what failed to improve negative behavior. By keeping this form in a binder or on your computer you can compile data so you can show parents and administrators what efforts you have tried and that something different must be done.
