I use this sheet to help students' correct their unwanted behavior.
They have a place to check how they thought their behavior was and what rule they broke. Next they describe their behavior, why the rule they broke was important and why they broke it. Then they write about what they could have done differently. There is a place for the student, teacher and parent to sign the form. At the bottom of the sheet is an area for the teacher to check off what punishment was given. You can edit this form to add your own rules and consequences.
