The behavior management journal with a QR code is a tool for teachers to use as a way of monitoring both desirable and undesirable behaviors in the classroom. The tool is a google form that is connected to a QR code. The Qr code is then embedded as an image on a student profile/contact page. I keep these profile sheets in a binder for each class. The purpose of attaching the google form to a QR code is to maintain quick access in submitting/monitoring student behavior or growth.
