This is a document to help correct undesired student behaviors. I normally don't give this out until after I've given 2 verbal warnings and I place this on their desk as a reminder of what will happen if I have to give a third warning, but use it how you wish!
It includes:
-"Fix-It Ticket" Page (2 or 3 per page)
-"Behavior Slip" Page (2 or 3 per page)
Both of the above pages include:
-Name/Date
-1st & 2nd warning checkboxes
-"Today I struggled with:" (w/ lines to write)
-"The poor choice I made was:" (w/ lines to write)
-"The next time this happens I will:" (w/ lines to write)
-Student Signature
-Parent Signature
-Behaviors Beware (list of expected behaviors card)
Expected behaviors include:
-Listening/Following Directions
-Talking at Appropriate Times
-Disrupting Others Learning
-Out of Seat without Permission
-Synergizing with Others
-Completing Work (school/home)
-On Task/No Horseplay
-Honesty/Kindness
-Talking out of Turn
-Controlling Your Body
-Respectful to Peers and Adults
*I explain this document in its entirety to my students at the beginning of the year and I send home a Behaviors Beware card so parents can also see our classroom expectations. I also print it on colored paper.
