A two-fold behavior note!
This is a great way to keep the parent's informed about their child's behavior AND for the student to reflect on their choices. The student should spend time in class to fill out this form, sign it, and give to the teacher for review. The teacher will sign the form, and send it home for the student to show their parents and return the next school day with their parent's signature to show they had a discussion with their parent.
I have created two versions.
- One consists of regular lines that would be ideal for 2nd grade and above. This version takes up less space on the paper and can be copied and cut into two halves to save paper.
- The other version has thicker lines with a middle line reference point for younger students. This version utilizes the entire page.
