Here you will find a Behavior Notification that I used in the classroom when I worked in the school system. This is given to families if a student continues to not follow directions and behavior appropriately in the classroom, I would send home this behavior notification to notify their family about their behavior. I tell families that this notification must be signed by a parent/guardian and returned the next day in the student’s Take Home Folder. It also gives a space for the teacher to explain the reason for the behavior notice and what actions the child will take to improve the behavior so it does not happen again!
