This is a behavior reflection form on google forms that I have students fill out when they receive a behavior card for the second time in my classroom.

The form asks the student to reflect on their behavior by acknowledging what they did and how they are going to prevent it from happening in the future. There is also a link to a respect agreement at the top of the form for students to understand what rule in the classroom they broke.

This is extremely helpful for documenting student behavior in upper grades
