Directions for Completing the Behavior Reflection Form
- Be Honest: This form is designed to help you reflect on your actions and find ways to improve. Take your time and answer each question honestly.
- Use Complete Sentences: Write in full sentences to clearly explain your thoughts and responses.
- Describe the Situation Clearly: In Question 1, explain what happened in your own words. Include important details but focus on facts rather than blaming others.
- Consider the Impact: In Question 2, think about how your actions affected yourself, classmates, teachers, or the learning environment.
- Think of Better Choices: In Question 3, reflect on what you could have done differently to handle the situation more appropriately.
- Plan for Improvement: In Question 4, write specific steps you will take to avoid repeating the same behavior in the future.
- Request Support If Needed: In Question 5, let your teacher know if you need any help, guidance, or resources to improve your behavior.
- Sign the Form: Once completed, sign the form to acknowledge your reflection. If needed, your teacher may also sign it.
