Description

This reflection sheet flow chart provides my students with an opportunity to self-reflect after a moment of frustration. It allows students to not only reflect on an incident or moment of weakness, but also think through where their frustration came, and what they can do differently next time. This document supports the mindset of goal-setting and finishes with an official signature from the student, the adult present, and the student's parents.

This behavior reflection is aligned to the ABC format:

  • Antecedent of Behavior (what happened first)
  • Description of Behavior (what was your reaction)
  • Consequence for Behavior (what should be done next)
