This editable behavior reflection sheet is a simple way to help kids think about their choices after displaying undesirable behavior. The sheet lets the child know they made a wrong choice, allows you to check the behavior from the list, and gives space for the child to reflect on what happened and what they can do better next time. Students then take the reflection sheet home for a parent signature and return it the next day. It’s an easy tool for encouraging responsibility, awareness, and communication between home and school.
