This short worksheet can be completed on paper (two sheets print per page to conserve your limited copies!) or as a fillable Google Doc and help students quickly but purposefully reflect on their behavior choices.
Students are asked to answer yes or no to whether their behavior solved the problem they were having and, if not (which is likely the case), what they need to do to move forward and what they can do instead next time.
This worksheet can be completed independently by students or can be done as a meeting with the teacher, counselor, or administrator. If students have difficulty with reading or writing, complete this worksheet by reading the questions to the student and writing the answers they share.
The digital version has space for teacher and administrator notes which allows you to use this as a behavior data collection tool as well. Let's help students move forward productively after a challenging incident of behavior.
