This behavior report card is perfect for those students on behavior intervention plans. First, determine which behaviors you are trying to improve (raising hand before talking, showing respect to others, staying awake in class, etc.). Create a behavior contract where the student fully understands which particular behaviors are being addressed. Depending on the grade level the student can receive either a happy face, sad face, +, or -. For grades K-2 if the student exhibits appropriate behavior for the entire class period place a :). If they do not, place a :(. I typically give students three chances each class period to choose the right behaviors. For students in 3-5, a + or - is given. Place this sheet in a bright folder for your student to carry with them throughout the day. Set obtainable goals and build from there. (Week one expectation: 25 +, Week two expectation: 28 +, and so on) Be sure to provide a great reward at the end of the day or week when goals are met! This works well for RTI.
