This behavior communication template is intended to document inappropriate, disruptive, or unsafe behavior incidents and help facilitate effective communication between teachers, students, administration, and even their parents/guardians.

I recommend sending it home for a parent signature and keeping the returned/signed form for documentation purposes and behavior tracking.

Keeping the forms allows you to look for patterns in reccuring behaviors.
