This is a basic behavior star chart that I have used in my Kindergarten classroom. More rows and columns can be added if you need. There is a visual reminder of the goals on the second page. This can be laminated, posted in the classroom, attached to a clipboard or desk space, or sent home.

This editable star chart has worked really well with a student of mine that struggles to stay in their seat. I like that it's broken up into the morning and the afternoon to make the likelihood that my student earns it more realistic.
