Behavior is communication. Children use behaviors when they don't have the words to communicate what they are feeling (whether they understand what is happening or not).

Use this form to:

*find out what is triggering the child to cause the behaviors

*gather the data you need to get further support for the child (sharing the data with professionals)

*be able to describe in observable terms to parents and staff what the behaviors are and what may be causing them.
