Included are the following:

- a "simple" goal sheet: this sheet has a straight forward format for students who already know how to create an attainable, relevant, and clear goal for themselves

- a "strategies" goal sheet: after creating a goal, this sheet requires students to consider and record strategies that will help them achieve their goal

- a SMART goals sheet: this sheet gives students more prompting to follow the SMART goal process to create their behavioral goal
