A checkout list for your classroom library. Post it up on your library, or keep it in a binder behind your desk. I've used it in my 7/8th grade classroom as a chance for my students to be responsible for themselves. It also allowed me to check-in and see how the book was for the students and create a building block for my relationships with them.

Considered 2 pages, but you can add more lines, or simply use the first page over and over again.
