This document can be used with or without the CHAMPS program, but it is extremely useful either way, especially for older grades where the teacher is trying to create a sense of responsibility in their students.

Make it a point in your classroom to have every student turn in something at the time when an assignment is due. If a student does not have their assignment (for whatever reason), have them fill out one of these forms and turn it in in the place of their missing work. This form has a place for students to "plead their case" and you can decide if extenuating circumstances create the need for extra time or if the student simply didn't do the work. It will also be a way for you to show parents just how many assignments a student misses... (it's ALL about the "paper trail"). It will also put your mind at ease that you haven't misplaced so-and-so's homework. Great for Junior High and High School students!!
