I use this log on my students desks to keep track of where they are in their chapter book. During AR time I walk around and have the students show me where they are in the book and sign off on their sheet. Once the test has been taken we put their log in a plastic sleeve in the back of their AR folder so they can be reminded of how many books they have read!

(Helps us to keep track if they are actually reading or not!)
