This form meets requirements for Check In/Check Out. Check In/Check Out is a wonderful Tier II intervention for students. This form allows an educator and parents to track student behavior that is aligned to specific goals. These goals were created by analyzing student data and each goal is aligned with a PBIS expectation. Student behavior is rated by a score of 0, 1, or 2 for each class period. All scores are totaled and the student must meet their daily goal. For example, 80% of the possible points for each goal. This form also provides a space at the bottom of the page for the student to write a recognition/reward they are working toward at the end of the week by meeting their goal percentage. The outcome data is extremely helpful in tracking target behaviors and can also be used for IEP goals. Goals may be removed if a student masters a certain goal over an extended period of time. For example, the student meets the expected goal for 9 weeks. This daily form also allows for parent engagement and communication between school and home to occur. This resource can be life changing for at-risk students and staff.
