This *EDITABLE* chart is an example of the research based Check in, Check out behavior strategy that I use with my students throughout my school building. This allows easy data collection for MTSS data, IEP data, and can be used to address a multitude of behaviors including task avoidance, aggression, and attention seeking behaviors. Examples of goals are included as well as a rubric to assist other teachers in your building with scoring the student consistently.
