Check Yourself contains four forms that can be useful in classroom management and documentation. How I Roll form is a simple checklist that helps students describe the environment that they need achieve academic success. Student, parents, current teacher or previous teacher can complete this form. A student completes the Behavior Reflection form after a rule infraction. A student completes a Reporting and Incident form after observing an incident that needs to reported to an authority figure. Level of Mastery form allows students to monitor a specific skill(s).
