I use these request slips to avoid interruptions by students. There are three slips on one page. I copy them on colored paper and cut them in strips. One is for late work, one is for making up a test or quiz, and one is for general needs like grade corrections or signatures needed. Student know to complete the slip and turn it in. These have helped me a lot!
