1st Grade Common Core Standards - Documentation Forms

LESSON PLAN FORMS - Language Arts & Math:
- At the top of the first page, write the assignments or activity.
You will circle: M T W TH F (day of the week) for the days that
you teach that skill. There is room to write 6 assignments. In the
charts below, you will mark the Common Core standards for the
lessons or activities listed at the top of the page.

PROGRESS REPORTS - Language Arts & Math:
-There are 2 different forms. The rating system:
0 = No evidence 1 = somewhat 2 = needs improvement
3 = on grade level 4 = exceeds grade level

-One of the forms has the numbers 0-4 written in the column so
all you need to do is circle or highlight the number.
-The second form has blank columns so you can write notes.

DOCUMENTATION FORMS - Language Arts & Math:
-This form is open-ended so you can write notes beside each
Common Core standards.

ZIP FILES:
