This is a page that can be used with book assignments. It can be used as a way to standardize the way students turn in the assignments and it simplifies your grading. Just write the lesson, page numbers, and problem numbers at the top. The page is divided into sections the top section is for the skill level problems, then a simple application problem, then at the bottom a real world application problem, and an error analysis or critique problem. This type of format for homework seems to work well with a CCSS class or any class working to develop Higher Order Thinking Skills. You can adapt the page to make it work for you. I label the lines with the problem numbers they are suppose to complete and add my own answer line under each one at the top, then they can use the boxes for the other problems, but experiment and see what works best for YOU!
