I put this communication log in my student IEP file on the left-hand side. I use it to record the parent’s name, email, contact number, and preferred method of contact. I also have a copy for those students who have Guardians instead

In the section below, I write down the day of communication, the method, the purpose, and who I was in touch with.

I love this form because I have all my communication from parents and teachers logged in one place. Then I can go look up my notes or printed copies of email to review the communication.
