Description

Since Transition students are often out in the community on a daily/weekly basis, these forms are an easy way to prime students for community activities, as well as keep parents in the loop regarding the goings on of each day. It is a quick student inventory of what they have before the class heads out and provides opportunity for self-reflection at the end of the day.

Included:

· Community Checklist (Checking in with the student on logistics (ID, money, transportation, purchases made, etc.)

· My Day (personal check-in about where the student went, their activities, and favorite parts). I have found it to be a great thing to send home to parents to easily keep them in the loop.

· Community Day (a great day-before priming activity to prepare the class regarding what to expect, what time we are leaving, how we are getting there, etc.)

Each of these worksheets can easily be worked into a daily routine and assist the students in growing in responsibility, as well as giving you vital information before you leave the classroom setting.
