Copy and fill in a hard copy paper for each student before fall and spring conferences.  It really helps guide the conversation and makes sure the teacher is prepared with the scores and goals that parents want to see.  

I always make a copy of each of these after they are filled out as some parents like to take these home to review further.  This allows for me to still have a copy for my records too.
