Here is a form I use as a Confidential Reference Form. It can be used for Scholarships, Bursaries, Student of the Month/Year, or any other accolade. Used by teachers, counsellors, and administrators alike.

While it is designed to be filled in online, it can also be printed on a one page legal-sized piece of paper.

What my colleagues have appreciated in this form is that it is not onerous to complete, yet what I like is the essential information is all there in one place.
