This document is part of a bundle that was created for a parish 2 year Confirmation program for high school students. All students within the program were required to complete 10 hours of service a year. The form outlines how the 10 hours are broken down into family, Church, and community hours, how teens are record their hours, and gives ideas for what they can do for each area of service. The form has been motified to allow parish/school to enter in specific information for their school/parish. It also includes instructions on how to turn the form in, including a due date. This worksheet is used for both the year 1 and year 2 Confirmation program.

This document has been left in word format to allow for catechist/DRE's/teachers to alter the document as needed to fit their school/parish needs. The documents can work for both high school and middle school students, in a parish setting, or in a school setting.

The document is set up so it is printer friendly for double side printing.
