Description

This bundle includes 4 commonly used forms for school counselor documentation.

Session Notes
Use this one-page simple and clear forms to record notes during sessions with your students. This template includes space for a few life categories, a place to document scaling for any presenting emotion, reminders to use other forms for harrassment, risk and grade review, and guiding questions to help you through a session.

Report of Harrassment and Flow Chart

Use this one-page, easy to follow flow chart to process and document when a student reports harassment by another student. Also included is the Report of Harassment referenced in the flow chart. Great for documentation, keeping yourself to a process, and communicating with admin, parents and teachers.

Grade Review and Contract to Improve
Use these Grade Review and Contract to Improve to have simple, solution-focused conversations with students to address areas of need. This simple, but comprehensive form includes identifying reasons for the grade being where it is and defining strategies and a timeline for improvement. A page of Postive Learning Behaviors is also included to help the student choose new practices for owning their education.

This tool is not only a great outline and documentation tool for student conversation and goals, but can easily be photocopied or digitally shared with teachers and student.

Risk Assessment
*IMPORTANT: Risk Assessments should be facilitated by trained school staff such as school counselors and school social workers. This risk assessment is not a stand alone tool and in no way guarantees safety, so always communicate with guardians and mental health professionals when this tool is used. This serves as a collection tool to gather information to support students in need.

The last thing you want to be worried about when you are working with a child at risk is knowing if you are remembering to cover everything during your self-injury or suicide risk assessment. Use this risk assessment form to guide you through conversations with students to collect as much information as you can to move forward to support student safety. This form is a compilation of a variety of forms I've seen over the years, my own ideas and in a way that is organized to flow as a conversation.

Included in this purchase is a 4 page pdf document that includes suicide and self-injury risk assessment questions (including history, triggers, hope, goals, and more), parent contact guidance, and a safety plan template to compete with the student. This form was made to partner with the Columbia Suicide Severity Rating Scale, which is linked within the document in the course of the conversation where it is needed.

Please be sure to read the included terms of use before using this risk assessment form.
