Receive feedback from your students at the end of the year to reflect on your practices.
The students rate themselves and the teacher. Then, the course reflection asks the students what they liked most about the course and what lesson they would like the teacher to change. I also included a question about advice for next year's students--it's something fun to include in the beginning of the year.
I kept this file as a Word Document so you can add in the teacher's name and course name.
Remember, not every student will take this seriously. When asking for feedback from the students, you thick skin is a must! Overall, you should receive valuable feedback that will hopefully lead to tweaking how you do things the next go around.

My students really enjoyed this. Great resource!!!
Just wanted to share that this is an 8 question survey with 4 questions giving a 1-5 rating and the other 4 open ended. It would have been helpful to know that before I purchased. I was just looking for something with more components and directions for kids to do a longer term project or essay. if you´re looking for a survey students can fill out for homework or in a class period, this is a good resource for you.
Sorry to hear you weren't satisfied with this product! I thought I was clear with what it included--the students rate themselves, they rate the teacher, what they liked most about the course, what lesson they would change, and advice for future students. I also have it listed as a 1 page document.
