Description
This two-page small group lesson plan is what I use for my Daily 5 small groups. I write in my small group plans and keep them in a binder for easy reference while I meet with my groups. My class completes 4 rounds of Daily 5 during our reading block and then we finish up the 5th round at the end of the day during "at-risk" time. At Risk time is reserved to work with students who are in need of intensive intervention. By scheduling this way, I meet with every child during reading core, my kids who need extra support a second time in the afternoon, all while every kid in my class is actively engaged in Daily 5 activities! It is required for teachers to keep a record of what they are teaching/using/doing during at-risk time, so this lesson plan template is a great way for me to keep my records incase I ever need to reference what we have been doing in depth.

Love this idea and very thankful it is free, but wish it had a spot for the date, either the week or day.

Thank you for sharing!!
I love these! I use them to keep track of my different groups during small group instruction. It allows me to to write what I what rather than have designated spots.
