Managing behaviors in a classroom can be a very challenging and time consuming task. This behavior intervention tracking log can help you record and document student behavior in a way that makes it easy to turn behaviors into numbers based data. This behavior tracking log addresses the four main components of statistics, including frequency, duration and intensity of the behaviors and measures key intervention areas such as independence level, positive behaviors, behavior requiring intervention and progress toward IEP or Behavior Intervention Plan goals.
This system was developed for a non-public, full day special education school whose primary focus was to support student's behavioral growth. This form, however, can easily be implemented on a public school level and was created using an easy-to-edit excel file.
