Did you ever wish there was an easy way to document how a young student did at school or daycare? Have you ever had parents or other educators ask you how the student's day went?

Here is a handy tool for you to use to document how a young student did in the following areas:

• Reading & Phonics
• Behavior - on task, attentive, hands & feet to self, problem solving, appropriate voice level/tone
• Social Skills
• Fine/Gross Motor tasks
• Other general areas that pertain to your environment

There is also an area to write down something great that happened in the student's day as well. This resource would be most beneficial for young students in school, day care, early intervention, or therapy services. Can be used as data collection, general student documentation, parent home notes, etc.
