Description
This is a Daily Progress Note Form, a simple but powerful documentation tool used by teachers, therapists, or aides to track and communicate a student's daily performance, behaviors, and progress.
Purpose
- To provide a daily written record of how a student is doing in the classroom or therapy setting.
- To ensure consistent communication between teachers, families, and support staff.
- To track academic, behavioral, and social progress over time.
- To serve as a compliance document for students with IEPs, 504 plans, or scholarship requirements (like FES-UA, FTC, PEP).
Key Sections
- Student Name / Classroom: Identifies the child and their placement.
- Date / Teacher Name: Documents who wrote the note and when.
- Large Lined Section: Space to write observations, updates, and notes for the day. This might include:
- Academic progress (e.g., "completed 80% of math problems independently").
- Behavior (e.g., "had 2 verbal redirections during group time").
- Social interactions (e.g., "played cooperatively with peers during recess").
- Emotional regulation (e.g., "used coping strategies when frustrated").
- Homework reminders or parent communication.
How It's Used
- Daily Classroom Communication: Sent home with the student for parents to review and sign.
- Therapy/ABA Programs: Therapists document session goals and progress for accountability.
- Record Keeping: Provides a paper trail of interventions and progress for meetings, audits, or disputes.
- Family Engagement: Keeps parents informed on a daily basis, rather than waiting for weekly or quarterly reports.
