Description

This editable Google Sheet will help you better communicate student behavior with parents. The checklist has the following categories:

I ate my lunch

I didn't have any accidents

I worked hard on my reading work

I worked hard on my math work

I went to specials

I had occupational therapy

I had physical therapy

I had speech therapy

I worked on social skills

I had a good day!

These categories can be edited to fit the needs of your students. To edit a category, simply highlight the category and type in a more appropriate heading. To delete a topic that is not relevant, right-click the letter of the column that you wish to delete and choose 'delete column.' The sheet will auto reformat so that the title is centered.


This sheet can be used as a printable version, a digital copy for the teacher, or an interactive mode for you to update parents. The second tab at the bottom includes a printable version that will allow you to print 3 weeks per page to send home. To use digitally, simply make a new copy for your student each week. At the end of each day (or week), check the boxes that match your student's behavior, take a screen shot and e-mail or text to parents. To use this sheet as an interactive mode of communication, right click on 'student name' at the bottom of the sheet. Choose 'Duplicate'. Right click both tabs again, and click 'rename' and type in the date. Next, share the sheet with parents. In the upper right corner of the document, click the 'share' button. Type in the parent's email address. Each week, you will duplicate a new sheet so that parents can use the same link and always have access to the newest week.

I use this form differently depending on parents' preferred communication. For parents that are tech-savvy, I use the interactive mode. For parents that prefer paper copies, I will print.
